Throughout this course, you have examined counseling addiction concepts and strategies. In Weeks 2 and 5, you analyzed a case study client (Mary, Mark, or Anna) to critically think about and address common challenges clients may face on the path to sobriety. Now, imagine your case study client has successfully completed court-ordered substance use and mental health treatment. They are now ready to begin transitioning back into their community and/or daily life. The court has mandated a holistic treatment plan to address their transitional needs and support recovery. The treatment plan must include recommendations that address their career needs.

 

Your supervisor has requested that you review a discharge summary for your client and use the information provided to inform your decisions.

 

Note: A discharge summary is a document shared between health care providers such as a hospital, treatment facility, clinical practice, etc. Its purpose is to provide vital information about the status of a client, which includes details such as the purpose of treatment, procedures and assessments provided to the client, instructions for appropriate level of care, and the client’s current health status. The discharge summary is shared with relevant personnel as the client transitions from one health provider to another in the treatment process. A client is ready for discharge when they have met the respective provider’s treatment goals. In some cases, the summary can be created before a client is discharged.

 

Read your selected case study client’s discharge summary in the Client Discharge Plans document and reference it as needed to support your ideas. You may also choose to watch your client’s Discharge Plan video, which includes the same information.
