BSHS 415 Week 15 Field Experience Weekly Activity Log and Summary of Service Form

Review the Time Log Entry document on the College of Social Sciences Resources page for instructions on how to enter your Field Experience hours and submit your Weekly Time Log for approval.

Export your time log after your supervisor has approved the hours, and save the log as a document.

Once all of your hours have been submitted for the Field Experience, complete your Field Experience Summary of Service form through the Field Experience Module, accessed through the Week 15 Field Experience Assessment Form learning activity.

Submit your exported time log document and Field Experience Summary of Service Form to the Assignment Files tab. Ensure all forms have the required signatures prior to submission.
