Follow the exact format laid out in the sample reports found in Part I: Section 4 of Ekwall/Shanker Reading Inventory when writing your final diagnostic report.

This assignment–teaching a lesson one-on-one in a grade-appropriate classroom–requires a formal field experience evaluation. Remember to inform the classroom/site teacher that she/he will receive an email with the appropriate field experience evaluation form that she/he will need to complete. Ensure that you have an accurate email address for the classroom/site teacher. If you will be working with a client outside of a classroom setting (for example, working with a neighborhood child or a relative), please let your facilitator know.

Select “Field Experiences Evaluation – Instruction” in the Activity field on My Time Log to have the evaluation form submitted to your reviewer. After the evaluation is completed, be sure to export it and send it to your facilitator via a private message. Or, upload a copy of the exported file along with your final report and artifacts.

Submit your assignment.
