Why is it important to diagnose an issue before rushing to solve it?

 After reading the two chapters assigned this week, tell us how you have seen diagnosis approached in your organization, or another organization you are familiar with. Has it been effective or ineffective in addressing the issue it was meant to address? Use specific examples.
